How to Apply for the Lark's Prediabetes Symptoms Scholarship
Your application essay must not exceed 1,000 words, and your application must be in English. Any incomplete or incorrect submissions will not be considered. Lark assumes no responsibility for any technical problems with the submission form, or any internet connectivity or other problems that lead to late, incomplete, or incorrect submissions.
The deadline for scholarship application submission is December 1, 2018 at 11:59 p.m., ET. Late applications will not be considered.
Your application must be in English and must not exceed 1,000 words. One entry per person.
Eligibility for the Lark's Prediabetes Symptoms Scholarship
You must be a legal resident of the 50 U.S. States or the District of Columbia, or Canada, and at least 18 years old. You must be enrolled as an undergraduate or graduate student at an accredited college or university in the U.S. or Canada, or a high school senior (or graduated this spring) who is planning to enroll full-time at an accredited college or university in the U.S. or Canada in the 2018-2019 school year.
Lark employees and family members, and residents at addresses of Lark employees, are not eligible for the Lark scholarship.
Agreement to Terms and Conditions
Your submission of an application for the Lark's Prediabetes Symptoms Scholarship establishes your agreement to abide by the Terms and Conditions now and in the future.
Determination of Winners
Lark team members will evaluate each application and select the first, second, and third-prize winners based on criteria that include the quality and relevance of the essay.
Payment of Awards
Awards are for the amounts of $2,000 (first prize), $1,000 (second prize), and $500 (third prize). Awards will be in the form of a check made payable directly to the student. Scholarship awards are non-transferable.
Each scholarship award will be non-transferable and will be made in the form of a check payable to the eligible institution of higher education attended by the recipient. Winners must submit proof of enrollment to receive the awards.
Each winner will be notified via email and informed of their potential scholarship. Lark is not responsible for lost emails or emails that are not read by the intended recipient regardless of the circumstances.
Scholarship winners must provide proof of identity and any required tax forms and/or affidavits within 15 days of being notified of their selection. If they fail, or violate any of the Terms and Conditions, the award will be forfeited. Taxes owed by winners are their sole responsibility.
Potential Publicity and Promotion
Participation in the Lark's Prediabetes Symptoms Scholarship indicates your agreement that Lark and affiliates may contact you regarding the scholarship, and that your name may be posted on Lark websites, social media pages, and promotional materials, without compensation or notice. This includes name, voice, and/or likeness in advertising materials, Lark products and services, and the Lark's Prediabetes Symptoms Scholarship.
The applicant's essay and application become the sole property of Lark.
