Has anyone ever told you that you need to be a team player? That is true when managing diabetes, too. It is not only your responsibility but also that of your medical team. If you can work well with this team, you can win the game against diabetes. These are some of the key players on your diabetes team.

1. Physicians

Your physician is an integral part of your diabetes management team. It is important to attend scheduled visits and let your primary care doctor know if you have any changes in your health. Your doctor can help identify any needs for changes in medications and for referrals to specialists. Other physicians in the diabetes care team may also include experts who specialize in hormones, feet, mental health, heart and blood vessels, mouth, and eyes.

2. Nurses

Nurses working with your primary physician and others can help to coordinate your care to make it less confusing. Nurses are often the first person patients see at appointments and the ones who answer the phone if you have a question.

3. Registered Dietitian

A diabetes dietitian or RD is a friend in food. Not only can an RD tell you which foods are best to manage your blood sugar, but they can help you balance your diet or any other conditions you have, help manage your weight, and find tasty food you enjoy along the way! Your healthy diabetes meal plan can be tailored to your tastes and lifestyle.

4. Exercise Professional

The right exercise professional can be the clutch player on the team. The exercise professional can help you find exercises that work best for not only diabetes but also any joint, weight, heart or other special considerations you may have. They can help keep you motivated and stay on the top of your game. There is an exercise program for everyone, and your exercise professional can help you find one that you love.

5. Certified Diabetes Educator

A certified diabetes educator (CDE) is experienced in helping patients like you manage diabetes. Your CDE has passed a board exam and knows the ends and outs of daily diabetes management, such as how to fit healthy eating, blood sugar monitoring, physical activity into your life.

6. Lark

Who is there for you anywhere, anytime, to offer encouragement, tips, support, and reminders? Lark is available for compassionate coaching to keep blood sugar under control. The program is backed by our entire team of health and diabetes experts, who are all rooting for you to succeed.

With the combined powers of you and your diabetes care team, diabetes doesn't stand a chance of winning this game!
